Clive Barker's feature directing debut graphically depicts the tale of a man and wife who move into an old house and discover a hideous creature - the man's half-brother, who is also the woman's former lover - hiding upstairs. Having lost his earthly body to a trio of S&M demons, the Cenobites, he is brought back into existence by a drop of blood on the floor. He soon forces his former mistress to bring him his necessary human sacrifices to complete his body... but the Cenobites won't be happy about this.

After having her father and perverse stepmother killed by the cenobites, Kirsty Cotton is sent to a psychiatric hospital. Even after trying to convince the authorities of what really happened with her family, nobody believes in her version of the history and they decide to place her in the institute, so that she rests and relaxes a little bit. The hospital is commanded by a brilliant and strange psychiatrist, Dr. Channard, who has been looking for the key for another dimension for a long time. The only person who believes in Kirsty is a young and kind assistant of Dr. Channard, called Kyle MacRae. Following Kirsty's version of the story, Channard put his hands on the bloodstained mattress where her stepmother J�lia died, Channard decides to resurrect her, killing his patients and offering them as food for J�lia. She returns without skin, and she decides to help Channard to bring the forces of the evil, but the plans of the two will be disturbed by Kirsty, who wants to end at once with the cenobites, and for Kyle, who doesn't want to see Kirsty being hurt...

Hell is about to be unleashed again. Reporter Joey Summerskill ('Terri Farrell') slowly begins to learn about the mysterious puzzle box, the unbearable pain opening it can bring, and Pinhead ('Doug Bradley'), the Black Pope of Hell, after viewing a young teenage club-goer get ripped apart by the box's chains in an ER room. She tracks the box and a young woman named Terri ('Paula Marshall') down to a famous club called the Boiler Room, owned by the spoiled playboy J.P. ('Kevin Bernhardt'), who feeds Pinhead, trapped in a marble pillar, blood from club members so he can escape from his prison. As time runs out, Joey must think of a way to bring Pinhead and his newly-created Cenobites back to the realm of Pinhead's human self or succumb to an eternity of pain and suffering.

In the 22nd century, Dr. Paul Merchant (Bruce Ramsey) has created the ultimate space station for a special purpose: to destroy the Lament Configuration, which has brought silent shame to his family for centuries. In the 18th century, French toy maker Phillip L'Merchant (Bruce Ramsey) built the first puzzle box, which was used by magician de L'Isle (Mickey Cottrel) to create a beautiful demon named Angelique (Valentina Vargas), the daughter of Leviathan, the lord of Hell's Labyrinth. As the generations passed, L'Merchant's descendants began the work to create the Elysium Configuration - a box that would destroy the Lament Configuration, until 1996, when one of them, John (Bruce Ramsey) nearly fell to the power of the box through Angelique. Of course, Pinhead (Doug Bradley) and his Cenobites aren't happy about the fact that the gateway to Hell will by destroyed...

In London, after investigating crack addicted junkies for an article in her newspaper, the journalist Amy Klein (Kari Wuhrer) watches a bizarre videotape. Her editor Charles Richmond (Simon Kunz) received the footage of an underground group of youngsters in Bucharest apparently becoming zombies through the power of their leader Winter (Paul Rhys) from a member, Marla (Georgina Rylance), and invites Amy to prepare the story. Amy accepts the challenge, and once in Romania, she finds Marla dead with a puzzle cube in her hands. She brings the object to her hotel room, and opens it, beginning her journey to hell.

The youngsters Chelsea, Allison, Derrick and Mike are grieving the suicide of their friend Adam, who became obsessed with an internet game called Hellworld. Their former friend Jake blames the group since they have not stopped playing the game even when Adam was unstable. When they receive an invitation to a Hellworld party in an isolated mansion, the reluctant Chelsea decides to join the group and they surprisingly find Jake in the party. He explains that he was invited by a girl he met in a chat room. They are hosted by the owner of the place, who shows them his private macabre collection. Along the night, they find that they are in a party in hell.
